Connect & Care is a comprehensive, web-based, communication platform. If you can access the web, you can access your Connect & Care account.

Desktop Computers

Once you are provided your Connect & Care account, you can access the Connect & Care from your desktop computer just as you would any other password-protected site such as your bank account or favorite online merchant.

Smartphones & Tablets

Connect & Care has iOS and Android smart phone apps as well. These apps will allow you to access the Connect & Care platform, communicate with your care team, and receive notifications when there are updates, messages or issues that need your attention.
